CATASTROPHIC CASE MANAGEMENT
According to Canadian Standards of Practice,
in 2009 the National Case Management Network (NCMN) defined Case Management as a
“collaborative, client-driven process for the provision of quality health and support services
through the effective and efficient use of resources. Case Management supports the clients’ achievement of safe, realistic, and reasonable goals within a complex health, social, and fiscal environment.”
This definition describes a complex, multidisciplinary process
that requires the input and support from many players in the health care delivery system, including:
~ The client
~ The client's support system, such as family and friends
~ All care providers
~ All vendors and / or other suppliers to the health care system
All case management is conducted in a coordinated way to achieve agreed-upon goals for the individual.
Clients who require Case Management are often experiencing acute or chronic health and / or social issues that necessitate a wide variety of resources and services. The relationship between the client and case manager may be short- or long-term.
